Admission Requirements

• A face to face assessment of the potential resident
   
• Documentation of completed TB test or chest x-ray taken within 90 days prior to admission.
   
• Medical chart review (if hospitalized or in a skilled care facility)
   
• An Interagency Referral Form and discharge summary (if being admitted from a hospital or skilled care facility)
   
• Signed physician’s orders including medications,over the counter medications, treatments, diet, activity, and an order to administer / supervise the resident’s medication administration by staff.
   
• A financial assessment by Calumet County Dept. of Human Resources 920-849-1400 or e-mail: HumanSvc@co.calumet.wi.us

• Laundry basket
• Clock
• Cable ready television
• Wastepaper basket for bathroom and/or kitchen
• Extra bed pillow and pillow case
• Framed pictures and knick-knacks, if desired
• Clothing, labeled with your name
• Bulletin Board
• Coffee maker
• Toaster
• Dishes or silverware if you desire to eat in your room
• Up to $200 to start a petty cash account
• Copies of Power of Attorney,Advanced
• Directives and insurance cards
• Toilet riser, wheelchair,walker or other assistive devices, if needed
• Incontinence products, if needed (we can also order from pharmacy)
• Glucose testing kit, if needed
